Covid19

Covid19 may soon overwhelm most national health systems, with a heavy death toll due largely to a lack of ventilation systems. As of today, March 25, 2020, Italian physicians must already take the decision as to which patient can access a ventilator, based on patient age. Yet Italy is one of the top 10 health systems worldwide! Reference Triage on wikipedia.

I have the opinion that to increase the number of available ventilators we have no choice other than to build them from components ready at hand. This website intends to provide examples how to do that

Motivation

Wir haben Montag 23.3 auf Heise eine Einladung zu einem Wettbewerb der McGill Uni, Montreal, CA gelesen: McGill hat 200.000 CanDollar ausgelobt für die Einreichung von kompletten Fertigungs, Einstell- und Bedienunterlagen für leicht reproduzierbare Atemgeräte.

Grundsätzlich eine exzellente Idee, aber sie krankt an der gleichen Stelle wie die Bitte des Ministerpräsidenten von BW, Herrn Kretschmann, an die Autoindustrie, sie möge bitte Beatmungsgeräte bauen: Wir machen erst mal einen genauen Plan, wenn der fertig ist und der Produktionsprozeß langsam hochfährt ist die Infektionswelle durch und die Leute tot. Herr Trump versucht das in USA derzeit auch. Die Autoindustrie hier hat inzwischen abgewunken.

Die Ausschreibungsbedingungen von McGill sowie vermutlich die Ansätze der Autoindustrie haben einen entscheidenden Makel : Sie verlangen einen genauen Plan! Keine Abweichungen ! Wenn die Teile alle sind, sind sie alle, Ende der Produktion. Sorry you're dead.

Ich denke, daß die Maker-Szene noch eine Chance hat, an dieser Stelle massiv zum Besseren zu wirken. Wichtig ist IMHO, daß man kein festes Design fordert, sondern die Atemgeräte nach Verfügbarkeit von Komponenten aus einer Palette von Möglichkeiten zusammenbaut, je nachdem, was verfügbar ist.

Wir haben nach der Lektüre des McGill Artikels Montag festgestellt, daß wir einen Haufen Ideen haben, wie man einfache Atemgeräte bauen kann. Danach konnten wir nicht zurück zum Tagesgeschäft überwechseln: Man hast ne Lösung für ein weltweites Problem und will das ignorieren ? Geht nicht. Das Geld interessiert uns nicht.

Ziel

Unser Ziel ist, auf einer Website Baupläne, Konzepte, Kalibrieranleitungen, Bedienungsanleitungen etc. bereitzustellen. Die Maker in der Welt prüfen, was sie verfügbar haben, bauen mit dem was lokal zu haben ist Beatmungsgeräte, liefern diese an die Krankenhäuser und trainieren das Personal im Umgang damit, schreiben zur jeweils ausgeführten Version passende Bedienungsanleitungen in lokaler Sprache. An uns zurückfließende Ergebnisse werden aufbereitet und veröffentlicht.

Erste Zielgruppe dieser Seite sind Maker, die zusammen mit Studenten der näheren technischen Universitäten ermitteln, was lokal vorhanden ist oder schnell beschafft werden kann. Fehlende Komponenten können von lokalen Werkstätten, 3D-Druckern, CNC-Werkstätten, Acrylglas-Lasershops je nach Bedarf entwickelt und gefertigt werden. Angehende Ärzte und erfahrene Anästhesisten prüfen die Beschaffbarkeit von Verbrauchsmaterial in diesem Bereich : Beatmungsschläuche, T-Stücke mit Tubusanschluß, Rückschlagventile, PEEP-Ventile. Was käuflich erwerbbar ist, muß nicht gefertigt werden. Seid kreativ!

Stellen sie sich eine zum Covid-Spital umfunktionierte Schule in Südkenia vor, wo der Tauchshop einen Tauchkompressor für Reinluft und einen Stromgenerator ranschleppt, die Maker haben die Beatmungsgeräte zusammengebaut, aus irgendeiner stillstehenden Fertigungsanlage haben sie die Ventile "ausgeliehen", ein AccessPoint findet sich, irgendwer hat 3 Android-Smartphones aufgetrieben, die jetzt mit einer passenden App die Beatmer der Station überwachen. Redundant. Ein Maker bleibt als "Feuerwehr" im Spital.

Solange Leute, die sonst liegengeblieben wären, nach der Behandlung mit solchen Geräten wieder aufstehen, ist das ein Erfolg. Nach Nachrichtenlage bekommen schon heute 75-jährige Patienten im Elsaß wegen Gerätemangel Morphin statt Druckluft !

Die nächsten Wochen, auch schon Tage, sind entscheidend dafür, wieviel Beatmungsgeräte man vor der Welle in die Fläche bekommt. Geschwindigkeit ist derzeit die wichtigste Ressource.

Medizinische Aufgabenstellung

Wegen Covid-19 beatmungspflichtige Patienten sind i.d.R. schwerst krank ! Unser medizinischer Berater, Prof. Dr. Rossaint, Uniklinik Aachen, faßt die Problematik wie folgt zusammen :

"Insgesamt muss einem bewusst sein, dass es bei diesen mit Covid-19 erkrankten Patienten um eine äußerst komplexe Intensivbehandlung geht; es ist also aus Sicht des Mediziners nicht nur ein Beatmungsproblem vorhanden, sondern die Patienten haben in der Regel eine sehr schwere Sepsis mit weiteren Organdysfunktionen.

ich habe nochmals kurz zusammengefasst, was die medizinischen Minimalanforderungen an ein einfaches Beatmungsgerät sind:

  1. Es muss in der Inspiration ein Druck zwischen 25 und 35 cmH2O (peak pressure) aufgebaut werden können; normalerweise soll der Peak-Pressure < 30 cmH2O sein.. In der Inspiration werden zusammengefallene Lungenbläschen (Alveolen) wiedereröffnet.
  2. Es muss in der Exspiration ein positiv endexspiratorischer Druck (PEEP) zwischen 10 und 20 cmH2O gehalten werden können. Der PEEP verhindert partiell das erneute Zusammenfallen von Alveolen.
  3. Es muss sichergestellt werden, dass das Inspirationsvolumen (VT) nicht mehr als 6 ml/kg ideales Körpergewicht beträgt. Optimalerweise ist es zwischen 4 und 8 ml einstellbar.
  4. Es muss sichergestellt werden, dass die Differenz zwischen end-inspiratorischem und end-exspiratorischen Druck nur 14-15 cmH2O beträgt.
  5. Der Inspirationsflow sollte 45 l/min nicht überschreiten.
  6. Das Verhältnis von Inspiration zu Exspiration sollte in etwa 1:1 betragen.
  7. Die Atemfrequenz sollte regulierbar zwischen 15-25/min sein.

Dies sind die Grundvariablen, mit denen wir bei einem kontrolliert beatmeten Patienten in der Frühphase des akuten Lungenversagens (ARDS), was bei den Covid-19 erkrankten Patienten vorliegt, beatmen (siehe beiliegende Präsentation zur Pathophysiologie und Beatmung des ARDS).

Will man nicht nur die initial schwierige Situation überbrücken, sondern denkt auch an die Phase der Entwöhnung vom Beatmungsgerät (ca. 30-50% der 2-4-wöchigen Gesamtbeatmungsphase), so muss die Spontanatmungsbemühung des Patienten erfasst werden können und das Gerät muss die Spontanatmungsbemühungen erkennen und unterstützen können. Hierfür werden üblicherweise Flow- oder Drucksensoren genutzt, die sehr genau einen minimalen vom Patienten generierten Flow oder Druckabfall (=Inspirationsbemühung) erkennen. In dieser Situation unterstützt dann das Beatmungsgerät mit einem entweder druck- oder volumenkontrollierten Atemhub."

Einsparen von Ressourcen

Der Demonstrator auf Beatmungsbeutel-Technik nutzt derzeit zwei Magnetventile, eines zum Einatmen und das andere zum Steuern der Ausatmung. Durch diese Trennung kann der Zeitpunkt, ab wann im Atemzyklus ausgeatmet wird, frei bestimmt werden. Verzichtet man auf diesen Freiheitsgrad, beginnt also die Ausatmung unmittelbar nach Ende der Einatmung spart man ein Magnetventil, weil beide Funktionen auf einen Kanal gelegt werden können. Der Unterschied könnte, wenn Ventile knapp sind, die Anzahl der baubaren Beatmungsgeräte je nach Blickrichtung halbieren oder verdoppeln.

Wenn zuverlässige Modellbauservos in hinreichender Menge vorhanden sind, können diese Magnetventile ersetzen. Im Demonstrator könnte ein Servo, das während der Einatmung das Loch im Druckgefäß mit einem Stopfen verschließt das pneumatisch release valve ersetzen.

Wenn genug Luft da ist, könnte diese konstant - ohne Ventilschaltung - fließen und das Gerät käme mit einem einzigen Modellbauservo aus.

Keine Ventile, keine Servos : Es gibt billige Zugmagnete. Kombiniert mit einer Feder und einer vom Magneten gezogenen Gummiplatte könnte das Loch im Druckgefäß geöffnet und geschlossen werden.

Numeric simulation of pandemic

Epidemic2.xls is an Excel spreadsheet containing a simplified numeric model of the pandemic based on german case counts : On the left are the daily totals issued by the Robert Koch Institute;

This model is editable : In cells in the upper area :

  • The expected portion of the new cases of the day needing intensive care
  • The average number of days a case blocks one intensive care place.

For every day you can modify

  • The epidemic multiplication factor (Number of cases today / number of cases yesterday). Lower figures here reflect success in social distancing / reducing the spread. a 1.0 here means further infection si stopped.
  • The number of intensive care beds to reflect successes in providing additional ressources

Here in Germany we may have the means to reduce this spread. Other countries simply will not cope.

Motivation

Having read about the challenge set by the McGill University of Montreal, CA, to design a simple ventilation machine, I am of the opinion that exact design drawings may well might produce good results, but at too late a date. I therefore developed a ventilation system utilizing as many existing components as possible. Although only at the beginning of the development, I have made considerable progress - but need every assistance.

Goals

This website is dedicated to publish concepts, ideas, schematics, technical drawings, sourcecode of components usable to build ventilators.

The world's maker community, together with students and staff of local universities identify which technical components are available locally and what cen be bought. Students and staff of medical universities likewise in their field (a PEEP-Valve I can buy is better, safer and faster to get than a homebrewn one). From available material they build ventilators and help their communities through the pandemic wave. Technical descriptions, translations, operation ans calibration instructions are fed back to us and included into information offered here

machine workshops, factories unsing CNC, 3-D printer owners, acrylic laser stations build the parts not readily available

Imagine a clinic in rural Kenia where a local scuba diving station provides a compressor, a car repair shop brings a power generator. Makers in Nairobi have built some ventilators, with solenoid valves borrowed from a factory. Someone brings an access point to connect the ventilator controllers with some android smartphones which are used for calibration and alarming. Some makers stay in the clinic to keep things running.

Every person who get back onto his/her feet who wouldn't have done otherwise due to treatment with those machines counts as success! In the french region Alsace patients over 75 are treated with morphine instead of ventilation (2020-03-27) !

Speed counts, the next weeks, even days are all whats left to do the planning and the building !

2020-03-27 : Today the demonstrator will ventilate a balloon ! Here are the components prior to assembly, from top clockwise :

  • pressure vessel, built from wastewater plumbing
  • pressure vessel output lid, also made from wastewater plumbing components. attached are the air supply tube with patient valve and balloon and the pneumatic controlled release or exhalation valve.
  • the solenoid valve island which translates electric commands to applied pressurized air.
  • the programmable logic controller (PLC) steering the valves
  • the inline air throttle regulating the ventilation speed
  • the pressure vessel input lid with the off-the-shelf resuscitator already attached
  • no pressure sensors (mentioned in the system chart) are used in this early stage
  • 202003-27 20:00 : The prototype works !

    View this video. Very basic, only timing and flow are controlled. The PLC is already replaced by a ESP8266 controlling the valves with a WIFI-attached Web server to control timing. no plateau phase after inhalation, no sensing of pulmonary pressure. That leaves me something to do for tomorrow ;-)

    The hissing sound is the air ejected through the patient valve. A balloon behaves differently than a human lung. You could not inflate the balloon if the the 60 mbar safety valve would be enabled.
